An athlete should complete this form with parent assistance/supervision as needed (for the U10 please try to involve your athlete as much as possible in the answering questions). The athlete will have the best overall input, as parents only see a small portion of what coaches provide. Parents miss the special bond that has been created with the athlete from dressing room, bench, and practice conversations, speeches, and positive motivation.
Please complete a separate form for each coach on your team.
The coach will never see this reference, and information within will only be shared for general information, in which case athlete/family names will NEVER be included.
